     Click on a document then you will be able to print it out. If you have any problems at all downloading any of these documents, please e-mail me and I will be happy to e-mail the document to you or mail it.

Bankruptcy Forms and Reports

     Intake Form, new client questionnaire in Adobe pdf format - use this form to itemize your debts.  I prefer that your questionnaire is mailed, e-mailed or faxed to me before your meeting, but if you cannot, simply bring it with you an appointment.  The evaluation of your situation is at no cost to you. Intake Form, new Client questionnaire in Word Format - if you don’t want to bother with Adobe pdf and you are using Microsoft Internet Explorer as your browser, you can download this intake form into Microsoft Word and print it out.

Proof of Claim form - if your employer files bankruptcy or if you need to file a claim for money owed to you, this is the form you can use to file  your claim. For cases in the Northern District of Georgia, you should mail an original and three copies of your completed claim form to Clerk,  U.S. Bankruptcy Court, 75 Spring St., 13th Floor, Atlanta, GA 30303.
